Job Summary:
As a Registered Nurse for our thriving PACE program, you will play a crucial role in providing direct and indirect nursing care to older adults. Working under the supervision of the PACE Clinical Services Supervisor, you will exhibit advanced nursing knowledge and skills in managing complex clinical situations to ensure the well-being of our participants.
Responsibilities:
- Conduct patient assessments and develop comprehensive care plans based on interdisciplinary input.
- Implement and manage patient care according to accepted nursing practice standards and organizational protocols.
- Collaborate with healthcare professionals to deliver prescribed medical services and ensure adherence to treatment plans.
- Record and evaluate patient care delivered, making necessary observations and adjustments when needed.
- Provide leadership by delegating tasks, monitoring patient care quality, and actively participating in quality improvement initiatives.
- Engage as a key member of the Interdisciplinary Team, contributing to patient care planning and staff meetings.
- Graduate of an accredited nursing school with a current RN license in good standing.
- Minimum of 3 years clinical experience, with preference for geriatric care exposure.
- Proficiency in paperless documentation is desired.
- Ability to work effectively in a team environment and communicate openly with colleagues.
- Basic computer skills and the aptitude to learn electronic medical record systems.
- Must possess reliable transportation, valid driver’s license, and adequate auto insurance coverage.
- Ability to pass comprehensive background checks, including criminal history and drug screening.
