Anthurium, PLLC
Anthurium is a small practice providing complex care, with complex scheduling and care planning needs. Services are provided in a concierge model that emphasizes top tier clinic management of complex illness, which requires a high level of professionalism and courtesy amongst staff.
The Nurse Coordinator (NC) will work full-time, Monday through Friday, from approximately business open to business close. Typical hours would be 8:30a – 5:30p. The appropriate candidate will have a background or strong interest in complex and chronic illness, and will demonstrate competency in core nursing skills, patient assessment, care planning, and independent problem-solving. Due to our small size, the ideal candidate is an experienced RN who is comfortable working with little direct supervision, and a willingness to improve workflows and protocols. The NC provides therapeutic patient and family centered care, serving as a liaison between patients and an interprofessional team within the clinic, and amongst patients’ expanded and often complex care teams beyond our clinic (e.g. primary care, specialists, lab services, pharmaceuticals, laboratories, etc.).
Salary commensurate with experience, in the range of $90k/yr to $115k/yr.
Clinical
- Proficiency in basic patient intake skills (e.g. rooming, vitals, pre-visit records review and preparation).
- Administer and/or monitor core clinical needs: lab draws (and processing), punch biopsy, adipose harvest, autonomic testing, transcranial doppler, basic wound care (and patient teaching).
- Learn the clinical needs of specialized services provided in-clinic (e.g. plasmapharesis, infusion treatments). Assist providers as needed.
- Experience and proficiency in: phlebotomy; infection control/prevention; IVs; fall prevention; hand hygiene; injections; seizure precautions; medication administration.
- Telephone, EHR/portal and in-person triage.
Review and act on test results, documenting as appropriate for ongoing care.
- Create and maintain patient registries as needed to improve patient care and clinical efficiencies.
- Assist with management of urgent and emergent patient care situations.
Care Planning
- Understand and communicate/explain/arrange the services provided in-clinic for patients.
- Ensure relevant advanced clinical testing is arranged, completed and received prior to in-clinic visits when necessary.
- Act as liaison between patients and pharmaceutical companies, ensuring agencies have the necessary information for dispensing & prior authorization, and patients have the information necessary for ongoing care.
- Oversight of IVIG ordering and treatment.
- Assist with scheduling of treatments and visits in-clinic as needed.
Patient Care Coordination
- Assist with the build and maintenance of EHR features that allow and improve patient care coordination needs.
- Assist in the creation and maintenance of patient registries that allow coordination of care for specific patient populations and services.
- Prepares charts and accounts daily as needed, verifying receipt of necessary records, treatment materials, and staffing.
- Anticipate and identify the advanced needs of patients, and act on them as needed (e.g. scheduling an interpreter, mobility needs, etc.).
Communication and Professionalism
- Communicate clinic safety, workflow and patient care needs to management and assist in improvements.
- Demonstrate high proficiency in patient needs and empathy for patient illness.
- Actively participating in clinic huddles/meetings as a primary constituent to clinic care and safety.
- Assist in the development of patient educational materials related to services, clinic expectations, and treatments.
- Role will require occasional errands outside of the clinic for urgent or atypical needs, which requires a valid driver’s license and access to a vehicle.
- Assist team members as needed with systems and resources used by and in-clinic.
- Serve as a mentor and resource to others.
Other Duties as Needed
- Incumbent will demonstrate nursing proficiency and independent leadership necessary to perform other duties as assigned or needed, which benefit patient care and clinic function.
