Registered Nurse – Case Management- Case Manager - Travel - (CM RN)
StartDate: 1/20/2025 Available Shifts: 8 D Pay Rate: $2723.94 - $3001.89
Responsibiities: Extended Care Coordinator, provide comprehensive case management for patient at skilled nursing facilities including coordination with SNF team members, nursing, rehab, discharge planning, dietary, admission, DON, administrator and KP physicians.
Required Qualifications
Certificates required: RN license
