BrightStar Care of S. Dayton/Warren County/West Chester® is looking for an RN Case Manager to manage the clinical care provided to our clients. The case manager accomplishes this by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches and monitoring staff performance.

This is a salaried position, working a flex schedule meeting the needs of our clients.
Required Skills/Abilities:
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Education/Experience:
- Active Ohio RN licensure, in good standing.
- 2 years of recent acute care experience in a home care or institutional setting.
- 2 years management experience not required. Responsible for supervising nurses and home health aides.
- Current Basic Life Support (BLS) certification.
- Valid Drivers License and Auto Insurance.
