CHHA Nurse

ur Home Care Registered Nurse will provide skilled professional service and case management oversight to a variety of home care patients.

We offer a 4-day work week option (8.5/hr shifts)
More quality time with patients 1:1

Responsibilities:
- Provide direct patient care in accordance with the individualized care plan
- Assist with activities of daily living, including bathing, dressing, and grooming
- Administer medications and perform treatments as directed by the healthcare team
- Monitor and document patient vital signs
- Assist with infection control measures to maintain a safe and clean environment
- Provide health coaching and education to patients and their families
- Ensure proper airway management for patients with respiratory conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date documentation of patient care activities
Skills:
- Strong communication skills to effectively interact with patients and their families
- Ability to provide compassionate and empathetic care to individuals of all ages
- Knowledge of medical coding and documentation requirements
