Position Purpose & Key Responsibilities: This position is responsible for providing a full range of Physical Therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment. Uses AIDET, perform hand hygiene, assists with positioning patient for comfort and pain management, and performs environment check prior to leaving patient’s home and maintains a clean and organized environment. Supervises Physical Therapy Assistants. Provides patient and/or family education to help achieve goals. Documents course of patient care including progress and continuing need for treatment. Participates in multidisciplinary meetings ensuring patient continuity of care.

Education: Graduate with Bachelor's, Master's, or Doctorate in Physical Therapy
Licensure: Current PT License through the AZ Board of Physical Therapy
Driver's License: Valid Driver's License, requires travel to patient homes
Certification: BLS through American Heart Association or American Red Cross
Clearance: Level One Fingerprint Clearance Card, within 20 days of hire
