Clinical Nurse Supervisor / Case Manager

Clinical Nurse Supervisor/Case Mgr , RN ; Supervises our home health care services that are delivered to patients in the comfort of their home in accordance with physician orders and a plan of treatment/care. Services are delivered by nurses, therapists, social workers and home health aides. The RN continues to implement standards, procedures and processes for delivering care that is safe, therapeutically effective, meets budgeted productivity goals and complies with all regulations. The RN is responsible for adequate clinical utilization to meet current and future demand for services. This individual oversees quality management and performance improvement programs with help of QA support.

JOB REQUIREMENTS
* 1 to 2 + years in a clinical leadership role at a Medicare-certified home health care agency .
* Experience managing skilled census of 30+ skilled service patients.
* Extensive experience with Medicare/Medicaid home health regulations is a plus. ( will be trained as well )
* Experience working with managed care organizations.
* Active and clear RN in Virginia or a compact state.
* Experience coordinating care from multiple disciplines.
* Proven ideas to reduce unnecessary hospitalizations.
* Experience with chronic disease management.

Possible Flexible Schedule is available.
Ample training is and support is available.

As a Home Health RN, Must have Experience with CMS/CHAP (Community Health Accreditation Program).
* Experience with Wellsky / Kinnser EMR is a plus.

Salary Pay: $85,000.00 - $95,000.00 per year
