Director of Hospital Case Management

Description

Job Summary:
As the Director of Hospital Case Management, you will oversee and coordinate all departmental
activities to ensure compliance with organizational policies and regulatory agencies. Your leadership
will drive the integration of patient care and discharge planning processes, optimizing clinical resource
utilization and ensuring continuity of care. You will manage daily operations, engage interdisciplinary
teams, and achieve established outcomes, all while maintaining an executive presence and excellent
communication skills.
Our client is part of a proud healthcare family serving the Denver area for over a century. With an acute care hospital boasting over 408 licensed beds, caring for more than 200,000 patients annually with the dedication of 2,000 employees, 500 volunteers, and over 1,400 physicians.

Team Structure:
- 2 Managers
- 30 Case Management Staff

Key Responsibilities:
- Direct and evaluate departmental operations, including staffing, information technologies, onboarding, and staff competencies.
- Allocate resources to meet productivity and quality goals.
- Oversee case management functions and ensure adherence to care coordination and discharge planning processes.
- Collaborate with the Facility CFO and Division CM Leader to achieve goals.
- Assess and improve department performance through data analysis and performance improvement activities.
- Ensure compliance with policies and standard operating procedures.
- Establish working relationships with key stakeholders including CMO, CNO, and ancillary service leaders.
- Perform other duties as assigned and adhere to the "Code of Conduct" and "Mission and Value Statement."

Requirements

- Bachelor’s degree in Nursing (required)
- Master’s degree in Nursing, Health Administration, or Business Administration (preferred)
- 3+ years of overall acute care hospital case management experience
- 2+ years in case management leadership in an acute care setting
- Previous HCA experience highly preferred
- Extensive leadership experience in case management is essential
