Homewatch CareGivers of Portland
Homewatch CareGivers is seeking a Registered Nurse at our Portland, Oregon location. Homewatch CareGivers provides personalized in-home care to support our clients’ unique needs. As the RN, you will manage client care services and provide guidance and oversight for our caregivers. We are a relatively small company, and our current delegation/taught task roster is small (less than 10). We service around 50 clients in total. We would like to find a motivated RN who wants to grow those numbers in tangent with our director of operations.
Should you join our team, your primary focus will be on managing the regulatory items required for in-home care and ensuring our clients receive quality care through regular visits with their care givers. To further this goal, you will be responsible for delegation and “taught tasks” for our caregiving team. You will be the primary contact regarding items specific to our client's care needs, i.e., doctors, care managers, etc. Lastly, the RN will be involved in the decision-making process for the feasibility of potential clients.
We are seeking a self-motivated, extremely organized individual who has a passion for teaching caregivers how to provide exceptional care.
Reports To: Director of Operations
Hours: Between 24 and 32 hours per work, with the potential to grow to 40 hours if desired.
Pay: $40 - $45, DOE
Benefits: PTO for all employees, medical, dental, and vision for FT employees (30+ hours a week)
Essential Responsibilities:
- Direct a preliminary intake of new clients and formulate or approve new care plans.
- Complete or direct routine visits to the client base to provide care plan updates if needed.
- Assess and chart observations of the client’s condition at each visit.
- Provide updates to case management monthly for clients with IDD.
- Coordinate with Physicians, PTs, OTs, and other individuals in the patient’s care plan to properly direct the caregiving team.
- Complete reevaluations for clients after any significant life event or hospital visit, per regulation.
- Complete delegations and taught tasks to caregivers to provide specific types of care to clients.
- Provide education to clients and families on proper home health care procedures and strategies when necessary.
- Provide education to caregivers through orientation and ongoing in-field advisement.
- Assist with direction to caregivers in the event of a potential client-related emergency. (This may result in calls outside of normal 8a-5p M-F from office staff informing you of an event. This is rare)
Qualifications:
- Able to work in the US
- 2+ years experience in the delivery of health care within the last five years
- Excellent problem-solving techniques and abilities
- Knowledgeable of the Federal Conditions of Participation and the statutory and administrative rules of the state’s regulatory bodies, to ensure compliance within the agency
- Ability to communicate clearly and effectively to staff
- Microsoft Word, Excel, and/or related experience
- Ability to pass a background check
- Current RN license in good-standing in the state of Oregon
