Job Description:
We are seeking a qualified Registered Nurse (RN) to join our dynamic team. This role involves providing direct nursing care to patients in their homes, collaborating with physicians, and coordinating with other healthcare professionals to deliver comprehensive care plans.
Responsibilities:

Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records.

Administer nursing care to ill, injured, convalescent, or disabled patients.

Advise patients on health maintenance and disease prevention.
*
Monitor, record, and report symptoms and changes in patients' conditions.

Operate and monitor medical equipment.
*
Help perform diagnostic tests and analyze results.
Qualifications:

Current RN license in the state of California.

Licensure and Certification:
* Must possess a current, unencumbered Registered Nurse (RN) license in the state of practice.
* CPR certification required; additional certification in home health care nursing is preferred.
Education:
* Associate or Bachelor’s degree in Nursing from an accredited nursing program.
Experience:
* Minimum of one year of nursing experience required, preferably in home health, acute care, or a related healthcare setting.
* Experience with electronic health records (EHR) systems.
Skills and Abilities:
* Strong clinical skills with the ability to perform comprehensive assessments and develop patient care plans.
* Excellent communication and interpersonal skills to effectively interact with patients, family members, and other healthcare team members.
* Ability to work independently and manage time effectively while maintaining attention to detail.
* Knowledge of home health regulatory and reimbursement requirements.
Physical Requirements:
* Ability to perform physical tasks such as lifting and transferring patients as needed.
* Must be able to travel to patients’ homes within the assigned geographical area.
