We are seeking a dedicated and experienced Family or Acute Nurse Practitioner to join our healthcare team. The ideal candidate will have a passion for providing high-quality care to patients across all age groups, with a focus on primary care. This position is working in a hospital environment and providing exceptional care. Your contribution as a valuable team member will be pivotal in ensuring the delivery of top-notch healthcare services. This position is a long term contract.
Responsibilities:
Conduct physical examinations and observe patients
Document medical histories and symptoms of patients
Create patient care plans
Keep track of any changes in patient’s health and adjust their treatment plant accordingly
Train patients and their families to manage and prevent illness and injury
Collaborate with the healthcare team to deliver comprehensive care to patients admitted to the hospital
Work closely with physicians, specialists, nurses, and other healthcare professionals to ensure coordinated and effective patient care.
Advocate for patients' needs, rights, and preferences while promoting a patient-centered and culturally sensitive approach to care.
Educate patients on preventive health measures and wellness strategies.
