Description
We are seeking a Nurse LPN or RN to oversee the health and wellness of our residents, in coordination with our Executive Director and House Managers. We care about your work-life balance and equip you with the tools you need to be successful at work.
Bachelor's degree NOT required
Knowledge & Qualifications:
At least 2 years of experience in Long Term Care/Assisted Living
Thorough understanding of DHS 83 and Nurse Practice Act
Licensed LPN or RN in the State of Wisconsin
Excellent leadership skills to maintain a motivated and stable team
Excellent organizational and problem-solving skills
Proficient communication skills
Responsibilities:
Provides assessments for potential residents to determine their eligibility for placement
Performs timely and accurate resident assessments, communicating changes to the Administrator for billing purposes
Creates customized Care Plans for each resident, ensuring team members have been properly trained to execute all tasks associated with Care Plans
Provides oversight of medication management to assure all MAR's are correct and that the residents receive medications timely and accurately.
Ensures all charting is performed in a timely manner and in accordance with state regulations.
Creates positive team member relations through staff appreciation, recognition of excellent performance, and retention through ongoing and open feedback with team members
Ensures staff are fully trained, including nurse delegation
Ensure compliance with DHS 83, in partnership with the Administrator.

Ability/ willingness to perform all position responsibilities adequately and apply basic management principles of a Registered Nurse with a minimum of at least one year in home health or geriatric nursing in a similar position. Rehabilitation and restorative experience helpful. Graduate of an accredited school of nursing; current Registered Nursing License in the state in which you are employed.
