Part Time Licensed Practical Nurse (LPN)

Description

We are seeking qualified candidates for the position of Part-Time Licensed Practical Nurse (LPN) to work in our adolescent residential facility located in Ridgewood, Queens. The PT LPN is responsible for the delivery and implementation of patient health services and serves as a member of the treatment team.
Schedule:
Saturday & Sunday, 7:30 AM to 3:30 PM
What You Will Do:
• Conduct and/or monitor all medication calls and health calls as scheduled.
• Work with clients and guardians to schedule medical appointments, assist in maintaining optimal health, and perform follow-up as required.
• Collaborate with the Agency psychiatrist regarding client medication and pharmacy refills to ensure prescriptions are filled/re-filled promptly and communicate medication changes to guardians.
• Provide on-site health services to address emerging or current health needs of clients.
• Offer training and guidance to key staff members to manage emergency health situations.
• In partnership with facility managers, establish and monitor health-related policies and procedures to ensure a safe and healthy environment.
• Provide immediate first aid in health emergencies and monitor the situation until emergency services arrive, assessing the situation. Escort clients to emergency or health facilities if necessary.
• Conduct health assessments (including nursing intake/health history, tobacco use history, cessation plan, and communicable disease risk) to make recommendations on participation in treatment and determine the need for physical exams or other health services.
• Attend physical exams when requested by the medical director.
Who You Are:
• Must be a NYS Licensed Practical Nurse (LPN).
• Experience working in a substance abuse treatment setting or with special populations is required.
• Experience working with adolescents is a plus.
• Bilingual skills are a plus.
• Must be able to work one of the schedule options listed above.
• Must be available to work occasional holidays.
• Flexibility in scheduling is a plus.
• Ability to collaborate effectively with diverse groups of people.
• Experience with AVATAR electronic health records is a plus.
