RN - Home Health

Responsibilities:

-Provide exceptional, patient-focused medical care in home settings.
-Assess patient needs and develop personalized care plans for optimal
health outcomes.
-Collaborate with healthcare professionals to deliver comprehensive
treatment and follow-up care.
-Educate patients and families on healthcare processes, treatment
plans, and wellness strategies.
-Monitor patient progress and adjust care plans to address evolving
health priorities.

-Valid Registered Nurse (RN) license in Pennsylvania.
-Minimum of 1 year of nursing experience, preferably in home health
care.
-Active RN License in the State of PA- American Heart Association
Basic Life Support (AHA BLS) or Advanced Cardiac Life Support (AHA
ACLS)
-Strong clinical assessment and critical thinking skills.
-Excellent communication, organization, and leadership abilities.
-Valid driver’s license and reliable transportation for local travel.
