Upstate Family Health Care
The Nurse Practitioner (NP) provides health services on an outpatient basis to patients of Upstate Family Health Center. The NP provides quality healthcare to patients in a clinic setting. The NP also functions as a team member of the agency’s medical staff and promotes financial stability, quality improvement, and community support of the UFHC and its programs. The NP job responsibilities will encompass preventative care, care of patients with acute and chronic illnesses, and education and guidance to support patients at UFHC.
Essential Roles and Responsibilities
Activities include but are not limited to the following:
Assess the health status of patients through health and medical history. Working collaboratively with the Physician, the Nurse Practitioner will carry out the major responsibility for health care needs including evaluation, diagnosis, management and follow up.
Institute and provide continuity of healthcare to patients, work with patients to endure understanding of and compliance with therapeutic regimen within established protocols and recognize when to refer the client to the supervising physician or other healthcare providers.
Provide instruction and counseling to individuals, families and groups in the area of health promotion, health education and maintenance, including involving patients and families in planning for their health care.
Work in collaboration with other health care providers and agencies to provide and where appropriate, coordinate services to patient and families.
Maintain clinical records according to established protocols.
Maintain clinical competence through continuing education, certification, and licensure.
Participate in planning programs and in developing specific health policies and procedures.
Practice safety, environmental and/or infection control methods..
Attends department meetings and any mandated continuing education/training programs
Comply with all pertinent regulatory and agency regulations and operating standards.
Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the Agency’s culture, values and mission.
Participate in activities as part of the team that may include meetings, trainings, and committees.
Other duties as assigned.
Education and Experience Requirements
Family Nurse Practitioner License (FNP) in the State of New York experience preferred, will train chosen candidate.
3+ years of experience
Evidence of essential leadership, communication, education, and counseling skills.
Proficiency in communication technologies (email, cell phone, etc.)
Highly organized with ability to keep accurate notes and records.
Local knowledge about the connections to community health care and social welfare resources is desirable.
Working knowledge of PCMH and the current requirements.

Knowledge of health education, practice and theory.

Knowledge of clinical operations and procedures.
Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise.
Knowledge of related accreditation and certification requirements.
Intermediate Computer Skills.
Must possess the ability to make independent decisions.

May be required to manage situations that require assistance of law enforcement.
May be required to work with individuals who may be hospitalized against their will.
