PURPOSE OF POSITION:
The Nurse Practitioner (NP) is an advanced practice nurse responsible for comprehensive direct patient management of the child/adolescent and adult. As an independent clinical provider and
utilizing the medical team as a resource, the Nurse Practitioner provides expert clinical care to a designated population of patients providing services but not limited to:
AREAS OF RESPONSIBILITIES
1. Conducts history, psychosocial evaluations and physical examinations
2. Analyzes laboratory results and follows-up on abnormal values
3. Monitor in-basket messages and take necessary action.
4. Assesses and treats acute illnesses according to established standards of care
5. Follows-up on chronic illnesses as a PCP has a defined panel of patients
6. Address preventative care and closes gaps in care for a panel of patients as clinically appropriate
7. Administers first-aid when necessary and is part of a facility emergency response team
8. Formulates a plan of care management for defined panel of patients including other disciplines as required to assist patients to a desired state of wellness
9. Completes patient correspondence (i.e. forms or letters)
10. Act as a liaison for external agencies and providers to answer questions and provide necessary follow-up.
11. Facilitate Home Care agencies requiring signatures, order clarification, changes, or request for additional services by collecting the required information and sharing with the patient's provider.
12. Communicates with pharmacies to answer questions about prescribed medications (e.g. duplicates, dosing change, etc.), refill requests, and non-formulary items.
13. Completes prior authorization for medications and equipment as indicated
14. Maintain accuracy and completeness of all patient records
15. Actively participates in departmental/facility/network in-services, meetings and trainings
16. Actively participates in the Medical Board/Department Committee meetings
17. Participates in Quality Improvement activities; develop and implement special projects (e.g. Quality Assurance Studies)
18. Acts as a preceptor for health practitioners in training, including medical students, residents, NPs, and PAs
19. Provides on-going supervision and/or evaluation of Medical Office Assistants (MA)
20. Maintains patient safety and infection control guidelines; follows established infection control protocols
21. Maintains data relative to Quarterly and Yearly Reports to the NYS DOH
22. Follows corporate and hospital regulations, policies and procedures
23. Fosters good customer service in daily activities and addresses patients, parents, and school personnel in a courteous and respectful manner
24. Maintains a professional working relationship with other health care professionals
25. Operates equipment and performs job related duties in a safe manner; supervises maintenance of equipment and supplies
26. Recognizes safety hazards and initiates appropriate action to eliminate hazards and prevent reoccurrences
27. Must be knowledgeable of Gotham Health mission/vision and uphold the mission in demonstrating the commitment to provide safe, high quality care to its patients.
28. Performs other duties as assigned within the scope of practice
Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
4. Board certification is required
Additional Qualifications
FNP and WHNP candidates preferred
2-3 years of experience as a practicing FNP preferred
