RN Hospice Case Manager

As an RN Case Manager, you will have the opportunity to facilitate the journey of end-of-life patients and their families through compassionate care directly related to the reason that our staff entered healthcare in the first place. Our team understands the mission and calling for hospice care and a successful candidate will share the heart for hospice. Often times, we have staff that enters a hospice career based on a personal event that touched them (family member passing, etc.) Quite honestly, resumes can look great, but we are concerned with the heart of the applicant. We are looking for registered nurses who desire to enter, re-enter, or join the field dedicated to providing their patients with dignity, integrity, and comfort. Our patients are residential and facility-based. For additional pay, our nurses will be on-call once or twice a week and one weekend per month. Full benefits available, mileage reimbursement, and on-call pay provided.

Registered Nurse in the State of Illinois. Proven experience in critical thinking/care (ER, Home Health, or Hospice). As stated above, we are looking for the heart of an individual. While previous hospice experience is preferred, being personally drawn to this field via past experience is acceptable.
