Description

The RN Staff Nurse shares the VISION of the Little Sisters of the Poor which is to contribute to the Culture of Life at Holy Family Residence. As a Home for the neediest elderly, each person is valued, treated with dignity and the wisdom of age is celebrated. The RN Staff Nurse is responsible for the provision and supervision of medical and personal care services in assigned area to assure residents attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with the comprehensive assessment and plan of care in conjunction with the Unit Supervisor. This position intentionally advances person-centered care and services irrespective of the presence of chronic and/or acute disease. These services are consistent with the Home’s standards and philosophy and are in compliance with all regulations codes, policies and procedures that govern the facility.

Essential Functions: Manuals, Policies/Procedures and Mission Integration materials for the Little Sisters are located in the Nursing Department. A personal copy of current Employee Handbook, the Home’s Supplement and Benefit Guide are provided at time of hire.
1. Assess and evaluate assigned Residents daily and report to relief nurse.
2. Timely reporting of changes of Residents status or condition to the Director of Nursing Services or the Director of Nursing Services designee and the Unit Supervisor.
3. Notify Resident physician and responsible party of change of Resident status, with prior reporting (if possible) to the Director of Nursing Services or the Unit Supervisor.
4. Assist Residents with issues related to psychosocial needs i.e. aging and health changes/illness, loss, and grief.
5. Administer and document prescribed medications/treatments accurately and timely and in compliance with policies and procedures.
6. Respond calmly and quickly to Resident and employee emergencies reporting to the Director of Nursing Services or her designee and the Unit Supervisor any emergency situations.
7. Transcribe accurately and timely all physician orders.
8. Participate in the comprehensive Resident assessment and update the plan of care in a timely manner to assure its accuracy and appropriateness.
9. Prepare for and assist with physician visits to the Home.
10. Assist with orientation of Nursing Assistants and/or select nursing staff as needed.
11. Supervise and follow-up of Nursing Assistants care and assignments.
12. Participate in the evaluation of Nursing Assistants as needed.
13. Report to the Director of Nursing Services or her designee and the Unit Supervisor any unresolved or pertinent issues with Nursing Assistants.
14. Assure follow-up of rehabilitation/restorative measures.
15. Communicate daily all pertinent Resident care information to Nursing Assistants, Nursing Staff, Director of Nursing Services, and the Unit Supervisor.
16. Communicate with family/responsible party regarding Resident issues/status as needed per instruction from or with the Unit Supervisor.
17. Document accurately and timely, including but not limited to accident/incident reports, admission/readmission, discharged, changes in Residents conditions/status.
18. Monitor medical supplies and equipment to assure proper use and safety keeping in mind cost and budget limitations.
19. Maintain confidentiality of all Resident, departmental, and organizational information. Demonstrate behavior that indicates recognition of Resident rights.
20. Work effectively and cooperatively as a team member within the department and with all departments.
21. Observe dress and conduct codes of Nursing Assistants, including identification badges.
22. Meet attendance standards as established and provide proper notice of absence.
23. Complete duties in prescribed time.
24. Adhere to all administrative and nursing policies and procedures, including but not limited to HIPAA, safety / OSHA, infection prevention, hazard waste (including Category I universal precautions), residents’ rights, quality improvement and assurance indicators and those contained in the employee handbook. Exhibit respect for each individual by assuring confidentiality of personal information and events.
25. Perform other duties as assigned by the supervisor.

Requirements

Requirements and Qualifications
1. Must be a graduate of an accredited School of Nursing, licensed to practice in the State of Minnesota as a Registered Nurse (RN).
2. A minimum of one (1) year experience as a nurse (preferred), preferably in the area of geriatrics, rehabilitation, or psychiatric nursing.
3. Current CPR/BLS for Healthcare Providers certification required. Must maintain active certification.
4. Must be able to function as a Charge Nurse ensuring that all nursing functions within the assigned unit(s) run smoothly and efficiently for his/her scheduled shift.
5. Must possess good written and verbal communication skills. Must have the ability to hear, understand, read, write, and communicate effectively in English.
6. Has evidence of good mental and physical health, maturity, and responsibility.
7. Must be able to function independently performing related duties/tasks with minimum supervision. Must also be able to function collaboratively.
8. Must have basic knowledge of local, state, and federal regulations and guidelines governing long-term care facilities.
9. Must have good assessment skills and a working knowledge of the nursing process.
10. Must be proficient in computer usage and software applications, including but not limited to electronic medical records.
11. Must possess supervisory and leadership skills to interact effectively with staff, residents, and families.
12. Must meet local, state, and federal requirements for background checks.
13. Possess the ability to perform essential job functions with or without reasonable accommodation.
