JOB DESCRIPTION
Position Title: Licensed Practical Nurse (LPN)
Job Summary:
The Licensed Practical Nurse (LPN) works under the supervision of a Registered Nurse (RN) to provide patient care, assist with assessments and care planning, administer medications and treatments, and supervise unlicensed nursing staff.
Key Responsibilities:
Patient Care:
Collects and documents patient information.
Administers medications and treatments as prescribed.
Observes patients and reports changes in condition to the RN.
Educates patients and families about care and discharge plans.
Care Planning:
Assists in setting patient goals and planning care.
Implements care plans and revises them when needed.
Supervising Staff:
Monitors unlicensed staff and ensures tasks are completed properly.
Collaboration:
Works with other departments for efficient and timely care.
Communicates with patients, families, and the healthcare team to ensure proper care.
Additional Duties:
Assists with physician rounds.
Performs shift-to-shift communication with nursing staff.
Follows safety and confidentiality policies (HIPAA).
Qualifications:
Graduation from an accredited nursing school.
Active LPN license from the State Board of Nursing.
Preferred experience in geriatrics or rehabilitative nursing.
Current BLS/CPR certification.
Physical Requirements:
Ability to stand and walk for long periods.
Lift, carry, and move objects up to 40 lbs.
Comfortable with tasks that involve reaching, bending, and climbing stairs.
Job Skills:
Knowledge of basic nursing principles and emergency care.
Ability to supervise and direct unlicensed staff.
Good communication and computer skills.
Graduation from an accredited nursing school.
Active LPN license from the State Board of Nursing.
Preferred experience in geriatrics or rehabilitative nursing.
Current BLS/CPR certification.
