Home Health RN - 8 HR Days - Bellin Health - Green Bay, WI
Radius Rule: Contractors must be outside a 50 mile radius of Bellin Health.
• Weekend/Holiday Rotation Requirements: Every other weekend, 1 holiday
• Float Requirements (campuses and/or units): Home health
• On Call/Call Back Requirements: None
• Required Licenses and Certifications: WI RN license, Active driver’s license. CPR certification (AHA BLS)
• Minimum Experience (yrs in specialty): 1-2 years home health
• Unit Overview: Nurses travel to patient homes or assisted living facilities to complete a comprehensive physical assessment, conduct diagnoses specific education, and provide hands on care which may include IV administration, IV site care, wound care, wound vac application, ostomy care, Foley insertion/care.
Nurses typically see 5-6 patients in an 8-hour day. Travel will be primarily local around the home health office in Green Bay, WI. Max mileage per day is 50, but more likely 25-30. Laptops are provided for required point of care documentation. Home health documents in the remote version of Epic (Called Dorothy).
• Unit Information (Bed size, Case types, Patient ratios, etc.):
We provide care to patients of all ages, from newborn to the senior population (Majority of patients are 65+). We care for a wide variety of diagnoses, and provide services to assist patients in their journey to healing, and health/wellness following an illness, injury, or surgery.
We have an incredibly passionate and talented multidisciplinary team consisting of registered nurses, physical, occupational, and speech therapy along with our social workers, and home health aides.
