Geriatric Nurse Practitioner Needed in PA

• Location: Coatesville, PA
• Dates: ASAP start, 6 month contract, options to extend contract
• Schedule: Mon-Fri, 20hrs a week
• Call: None
• Day to Day:
o Performs initial, semi-annual and annual assessments, performs additional assessments of participants as needed and implements appropriate plans of care.
o Participates in discharge planning for acute and long-term placement.
o Evaluate and treat participants during acute illness. Prudently prescribes medications, therapies and other treatments for participants. Following consultation with the Primary Care Physician refers participants to medical specialist as indicated.
o Make nursing home/or home visits when needed.
• Requirements: PA license, DEA, CSR, BLS, Board Certified by ANCC or AANP
• Experience: 5 years overall, 1 year working with elderly population is preferred
