Are you a passionate and dedicated Registered Nurse (RN) focusing on hospice care? Do you want to provide palliative and supportive care to terminally ill patients and families after normal business hours? Legacy Hospice is seeking an On-Call Registered Nurse/Case Manager with experience in home care to join our compassionate team.
Key Responsibilities:
Able to work autonomously with minimal supervision.
Perform on-call, scheduled, and admission visits during on-call hours.
Proficient with computers and electronic charting (EMR).
Assess and establish appropriate levels of care and continued hospice eligibility.
Revise the patient's plan of care with the interdisciplinary team.
Provide comprehensive care to maintain patient comfort and quality of life.
Teach and educate patients and caregivers about hospice philosophy and care.
Communicate with the Patient Care Coordinator regarding physicians’ orders and care planning.
Key Qualifications:
Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and Active CPR certification.
Valid driver's license, personal automobile, and insurance coverage.
One year of hospice care experience preferred.
Excellent observation, communication, problem-solving, and nursing skills.
Ability to perform physical tasks such as lifting, positioning, and transferring patients.
Effective social interaction, communication, and organizational skills.
