RN Case Manager - Home Health

Independence, LLC is looking for TWO Registered Nurse Case Managers who embrace the idea of expert care in the home and want to work in a collaborative team atmosphere where they can truly make a difference.
We are a family-owned, 14-year-old home health business positioned in the heart of central Indiana in a small community called Nineveh. We serve all of the major towns and rural areas within 45 to 60 minutes of our office.
The case managers with us are an integral part of the care we offer. We call on our clients regularly, making sure our clients are being cared for and that care is compliant with state regulations. We are seeking dedicated nurses who believe in our mission to help patients live our their best lives at home.
This career opportunity is primarily 8 a.m. to 4:30 p.m., Monday through Friday. While training is in the office, the opportunity is primarily an in-the-field position. The successful candidate will move from client to client throughout the day, completing calls and documentation while at the client’s home. All travel time from first client to last client is paid at the regular hourly rate and mileage is reimbursed.
Job duties include:
Job responsibilities to include comprehensive assessments including start of care, resumption of care and all comprehensive assessment time points that fall under home health.
Pay rate for this position:
$38 an hour

The following is necessary for consideration:
· Be a graduate of an accredited school of professional nursing.
· Currently licensed as a Registered Nurse in Indiana or holds a Nurse Licensure Compact (NLC) license recognized by the State of Indiana.
· Maintain CPR Certification.
· Ability to supervise and direct care team members.
· Knowledge of home health regulatory and reimbursement requirements.
· Home Care Experience preferred.
